Open Enrollment is just around the corner! You can make changes to your health benefits from October 27 – November 17, 2021 on workforcenow.adp.com. If you don’t want to make changes, there is no action required on your part – your benefits will roll over for the year 2022. Questions? Learn more here!


Does your situation qualify for a benefits change?

The most common qualifying life events are as follows:

  • Changes in Marital Status (i.e. marriage, divorce)
  • Number of Dependents (i.e. birth, adoptions)
  • Employee Employment Changes (i.e. going from on call to full time status)
  • Spouse/Domestic Partner or dependent employment changes (i.e. spouse job change which affects underlining coverage)

What do I need to provide Crestwood to change my benefits?

  • All qualifying life events require proof (which varies depending on the life event)
  • Additionally, your opportunity to make changes are typically 30 days from the date of the qualifying life event, so don’t delay!
  • To get the paperwork started please reach out to your campus/facility payroll contact or BOM.
